Bolt in Driver head dropped out...

Yesterday I wanted to change the shaft in my Mizuno MP Type 2 Driver, as said swapping adapter was easy trying to screw the bolt back into the adapter just wouldn't go in but why? I've had this problem with Titleist adapters but the Mizuno adapter worked fine as I've tried changing lofts a few times, I tried using a chopstick to push the rubber o'ring down the bolt as it looked like that's what needed to happen. End result is the bolt dropped out of the head and I've lost the o'ring as it dropped maybe onto the grass? Anybody have a solution to this problem I'd appreciate your advice....

I assume you are talking about the screw in the head that attaches the head and shaft correct?  I could be wrong but I believe the rubber washer is there just to keep the screw from falling out so losing it probably isn’t the end of the world. 
Now as to the screw not going into the adapter, have you tried to put the screw in the adapter while it isn’t in the head?  Perhaps some pictures would help as the only reason I can think of for the screw not to go back into the head is something is either inside the adapter or the screw is broken or both.

I agree with @GolfSpy_BNG, the washer is mostly there to hold the screw in the head but may also provide some kind of purpose like avoid overtightening and damage.  I would suggest ordering another screw and just taking the washer from the screw and using it:  https://shaftconnect.com/products/screw-compatible-with-mizuno-driver   Not sure that screw will fit your driver,  but I would think that almost any adapter screw washer would work acceptably.  

As for the screw not going into the adapter,  there could be multiple reasons with the most common being that  the screw got cross threaded or it isn't lined up properly.   I rotate the screw backwards to help get is aligned properly before tightening.    Again as suggested,  use the screw and make sure that it easily goes into the adapter.

The washers are easy to find. Putting them back on the screw inside the head is actually easy too, if you follow these steps

1. Get a Bic Stick pen and remove the tip so that you have the empty barrel.

2. wipe a thin layer of Vaseline on the open end of the pen barrel and place the washer on the end of the pen with the hole centered.

3. Insert the pen barrel/washer into the club hosel and press it firmly against the base (where the screw hole is).

4. While holding the pen barrel in there firmly against the base, insert the replacement screw through the hole and press until bottomed out.

It will go inside the washer and all will be well. As mentioned a few responses back, check the screw with the adapter before doing this to make sure it hasn't been cross-threaded and in need of replacement.

Yeah that adapter was slightly cross threaded so bought a Tap to fix the thread while I'm waiting for a new adapter, appreciate your feedback guys 😁
